Waiver and Release of Liability:

 

I, the undersigned, am the parent or legal guardian of the above-named child. I understand and acknowledge that my child will be participating in a birthday party event at Kicks For Kids, which may involve physical activities, including but not limited to Taekwondo.

 

I hereby agree to release, indemnify, and hold harmless Kicks For Kids, its owners, instructors, and staff from any and all liability for any injuries, damages, or losses that may occur during or as a result of my child’s participation in the birthday party event. I understand that Kicks For Kids will take reasonable precautions to ensure the safety of the participants.

 

I authorize Kicks For Kids to seek medical treatment for my child in case of an emergency if I cannot be reached. I also understand that the contact information provided may be used for emergency purposes and for marketing communications related to Kicks For Kids.

 

I have read and understood the terms of this waiver, and I willingly agree to its content.
